Thank you for choosing Monarch Lodge (Formerly Monarch Bear’s Lodge) for your accommodations while in Colorado. Please provide information below (please print) and initial and sign where requested. I agree to be responsible for myself and my guest(s) while staying at Monarch Lodge LLC and release the owners of any and all liability during my stay (see dates above). Myself and my guest(s) will leave the property in the same condition we found it, as instructed for departure. I understand that the $300 security deposit (attached and made payable to MONARCH LODGE) will be returned within 2 weeks of lease expiration, less any amount for damages or excessive cleaning.
A refund will only be given if notice of cancellation is given 21 days prior to arrival date (minus a $25 handling fee). If less than a 21-day notice of cancellation (for any reason), you are entitled to a refund only if the lodge can be rebooked for the same (or greater) amount of time reserved. A credit may be issued to be used within the next calendar year, at the discretion of the owner.
**Smoking inside cabin will result in forfeiture of security deposit.
**No refunds will be issued due to weather conditions or for circumstances beyond the control of the owners.

For refund, unused tickets must be returned to Cindy Hayek-Herron within 14 days of departure from cabin and no later than April 1st. Tickets must be in their original condition (both parts of ticket still attached) in order to receive refund.
